The First Medical Journal Theme Issues on Smoking

This exhibition is comprised of items related to Dr. Alan Blum’s editorship of the Medical Journal of Australia (1982-1983) and the New York State Journal of Medicine (1983-1985), during which he produced the first three theme issues at any medical journal about the world cigarette pandemic. The exhibition features complete scanned copies of the three issues, the letters to the editor and news media coverage in response to the issues, and a video interview about the obstacles encountered in producing the issues during an era of foot-dragging by organized medicine, academia, the mass media, and the public health community alike, fearful of attacking the politically and economically powerful tobacco industry. Also included are correspondence with the authors of the articles in these issues, letters from national and international public health leaders, reviews of The Cigarette Underworld (a book based on the first tobacco theme issue at the New York State Journal of Medicine), a 25th anniversary commemoration of the issues by the journal Social Medicine/Medicina Social, and correspondence related to the origin of the journal TOBACCO CONTROL.

“Cigarette Smoking”

For more than 40 years, beginning in the 1940s, Dr. Sackler (1913-1987) developed strategies to market pharmaceutical products to physicians. To this end, in 1960 he founded Medical Tribune, a weekly newspaper with a larger circulation (600,000 at its peak) than any other medical publication. While most medical journal editors of that era dragged their feet in condemning the tobacco industry and calling for greater anti-smoking efforts, Sackler published numerous news articles and editorials (including this one praising the theme issue of the MJA) aimed at reducing smoking. Sackler was a major patron of the arts, donating heavily to the Metropolitan Museum of Art, the Smithsonian Institution, and other cultural organizations. Since 2017 his name has been maligned by photographer Nan Goldin and others in the arts community for having marketed the opioid Oxycontin (oxycodone), even though he died a decade before that drug was developed and introduced. Few artists have ever questioned these same art museums’ acceptance of funds from Philip Morris and other makers of cigarettes, which take the lives of half a million Americans each year.

Lyle Stuart (1922 –2006) was an independent publisher of controversial books. In 1951, he launched a monthly newspaper, Exposé (later changed to The Independent), to publish essays that others would not have printed out of fear of offending advertisers or readers. Contributors included Upton Sinclair, Norman Mailer, George Seldes, Ted O. Thackrey and John Steinbeck. In 1956, with proceeds from libel actions against Walter Winchell, Confidential, ABC-TV, and Editor & Publisher, he began his publishing company, Lyle Stuart, Inc. After listening to an interview of Dr. Blum on New York City radio station WABC in 1984 about revelations about the tobacco industry and its allies in the New York State Journal of Medicine’s first theme issue on the cigarette pandemic, Stuart proposed adapting the issue for the general public as a book with a title that would hint at corruption: The Cigarette Underworld. Only one of the authors of the original articles in The Journal, New York cardiologist Richard Gorlin, MD, declined to participate. He had accepted research funding from the tobacco industry.

In the last week of December 1985, four months after the publication of the second theme issue on smoking, The World Cigarette Pandemic, Part II, by the New York State Journal of Medicine, editor Alan Blum, MD was fired by an interim executive vice president of the Medical Society of the State of New York (MSSNY), the journal’s publisher, with no notice and no reason given. His dismissal came within weeks after he was quoted in an Associated Press news story about the American Medical Association (AMA) House of Delegates’ passage of a resolution calling for a ban on cigarette advertising. While commending the AMA for its condemnation of cigarette advertising, Blum added that “the product with the longest shelf life in America is an AMA resolution.” MSSNY is a constituent medical society of the AMA.

CODA: “My sudden firing, coming just months after I’d been commended by the medical society’s House of Delegates for having revived The Journal and brought national attention to the Society, was a setback. Having been a full-time editor of peer-reviewed medical journals for five years, during which time I hadn’t continued patient care because I wanted to hone my editorial skills. I felt lost. But when I sought solace from a mentor, Kurt Deuschle, MD, chairman of preventive medicine at Mount Sinai School of Medicine, who had invited me to speak to the second-year medical students a few weeks earlier, he was not sympathetic. “It’s your own damn fault,” he said. “You got too far away from patients.” I heeded his advice, and after reaching out to two respected leaders in family medicine, Sam Nixon, MD, a past-president of the American Academy of Family Physicians, and Nicholas Pisacano, MD, the founding chair of the American Board of Family Medicine [then the American Board of Family Practice] (ABFM), I successfully passed my ABFM recertification examination and landed a position as an assistant professor in the Department of Family Medicine at Baylor College of Medicine, chaired by Bob Rakel, MD. Being granted the privilege of caring for patients again was the best thing that ever happened to me as a family physician. I learned, too, that when one door closes, another one opens.” AB

Origins of a new journal, Tobacco Control

After being fired in December 1985 at the close of his third year as editor of the New York State Journal of Medicine while in the midst of planning a third theme issue on the world cigarette pandemic, Alan Blum, MD shared with Surgeon General C. Everett Koop and Ron Davis, director of the Office on Smoking and Health, his idea for a new publication that would cover the tobacco industry, cigarette marketing, and anti-smoking efforts in the same heavily illustrated format as the theme issues he had edited at the New York State Journal of Medicine. Davis, a rising figure in the American Medical Association, presented the idea to the British Medical Association, publisher of the British Medical Journal, and in 1992 the concept became a reality with the launch of the quarterly journal TOBACCO CONTROL. Davis was appointed editor, and Blum became the deputy editor for news and feature articles.

CODA: “Three years after having been fired as editor of the New York State Journal of Medicine, I was thrilled to be offered the editorship of American Family Physician, the largest-circulation journal of family medicine. 31 others had applied for the job, and the process entailed four visits to Kansas City for interviews and an assignment to write an editorial. But did a double-take when I read that last paragraph of the contract and immediately called C. Everett Koop, MD, the U.S. Surgeon General, with whom I was planning two meetings of his Interagency Committee on Smoking and Health (on minorities and smoking and tobacco industry sponsorship of sports). I’d known that the AAFP was receiving funds for exhibitor booths at the Academy’s Annual Scientific Assembly from the food subsidiaries of Philip Morris (KRAFT and General Foods) and RJ Reynolds (Fleischmanns Eggbeaters) and that the Academy had had the same public relations firm as Philip Morris (Ruder and Finn), but I had no proof that this was a factor in the decision to muzzle my presentations on smoking. ‘You can’t take that job under those circumstances,’ Dr. Koop advised, and I turned it down.” AB
